You will be crucial in addressing patients' emotional, psychological, and social needs and contributing to their well-being Your responsibilities will include:
- Conducting assessments of patients' mental health conditions.
- Collaborating with the interdisciplinary team to develop individualized treatment plans.
- Providing counseling and support to patients and their families.
- You will advocate for patients' rights and ensure access to appropriate mental health services and resources.
Collaboration with community organizations and agencies will be essential to coordinate aftercare and support services for patients upon discharge.
Additionally, you will conduct crisis interventions and risk assessments to ensure the safety and well-being of patients.
Participation in multidisciplinary team meetings will allow you to provide input on treatment planning and patient progress.
Accurate and up-to-date documentation of all interactions and interventions in patients' medical records will be essential for maintaining continuity of care.
You will need at least a Bachelor's degree in Social Work from an accredited institution with prior experience in mental health preferred.
Strong knowledge of mental health disorders, treatment modalities, and relevant social work interventions will be crucial.
Excellent communication and interpersonal skills will enable you to effectively engage and support patients, families, and interdisciplinary team members.
